In order to schedule a consultation appointment (in person or a phone consultation) with Mr. Tooby you must complete a intake information form which will be reviewed by Mr. Tooby.

If he recommends a consultation he will set a fee which ordinarily starts at $1500.

When we receive the payment we will call you and set up a time to meet in person or by phone.

To prepare Mr. Tooby for the consultation and to best serve you, we suggest that you fax or mail us as many of the following documents as possible: (These documents can be obtained from the clerk's office or your original criminal attorney's file.)

• The charging paper (for example, the complaint, citation, or information.)
• The clerk's docket or minutes from the date the plea of guilty or no contest was entered.
• The clerk's docket or minutes from the date the sentence occurred.
• The abstract of judgment.
• The reporter's transcript of the plea of guilty or no contest.
• The reporter's transcript of the sentence.
• Any waiver of rights forms signed when the plea of guilty or no contest was entered.
• The police report.
• The probation report. (This may be difficult to obtain, but please try anyway.)

If you wish us to obtain the papers there will be an additional charge.
