After printing the form, please fill it out and mail or fax it to the address below with the following court papers:

• The charging paper (for example, the complaint, citation, or information.)
• The clerk's docket or minutes from the date the plea of guilty or no contest was entered.
• The clerk's docket or minutes from the date the report and sentence occurred.
• The abstract of judgment.
• The reporter's transcript of the plea of guilty or no contest.
• The reporter's transcript of the report and sentence.
• Any waiver of rights forms signed when the plea of guilty or no contest was entered.
• The police report.
• The probation report. (This may be difficult to obtain, but please try anyway.)
